Boost XT 16GB USB 3.0 PEF16GSBUSB "Unknown Device"

I hate to start on new forum with asking for help, but I got no where else to go.

I bought this flash drive on May 20 2012 and it worked great until today. I didn't use it that many times as I got 3 other 8Gb Patriot flash drives. Today I had this one on PC with Gigabyte motherboard (usb 2.0) and it worked fine for a while. I copied some files on it then moved it to my sever to dump those files but "Unknown Device" came up.
I am not sure is it dead 100% as it flashes when connected and found new hardware comes up, but then "Unknown Device"
I tried in on four computers, two on windows 7 Pro 64-bit and two on windows XP Pro 32-bit - same "Unknown Device" on all.

Is there any kind of program to revive this drive?

I got this one from newegg where I left my feedback on it as well as being a great flash drive

If anyone knows what to do please let me know.

Hello,

Can you please see if our low level format/reset tool will restore the drive for normal use? Please give it a try.
