SOLVED: help finding FRY'S rebate form for PBO CORE

I cannot post in rebates forums but I'm hoping someone has used this rebate form and can direct me to it. I originally saw this ad via Fry's email ad and had the form up on my screen in a PDF reader program before I went and bought it. At Fry's that night I found myself waiting at the counter for a price check on the PBO Player due to it being unlisted in the system? I saw it in their ad and even had the form but after many people checked out the plu numbers and upc's finally after 30 minutes or so after the store was closed they sold me one for $89, got home and my pc was not responding! I had to restart and lost that form, I eventually got it back on screen and was adding my info to the form so there would not be any errors in reading my form. I usually do this with a screen snapshot in Paint Shop Pro and add text to it in .tiff or other picture format then print out completed form. I have however been using a PDF reader that can edit forms and print them directly, not as easy as it sounds though. This form was quite strange while adding text it wasn't cooperating and eventually I gave up and was just going to go back to the PSPro method, but I couldn't find the link any more? I also couldn't find the ad listing any more either? I've searched everywhere and it's not on my comp nor is it online? It was in my email ad for Feb 18 ad week. I believe it was a $35 or $30 rebate. The Patriot rebate center has no listing of a Fry's PBO rebate for that date, Fry's shows no such product even for sale at all now. WTF? Never had this happen before in 100's of rebates I've done. It wasn't a dream! Any help pointing me to this form would be greatly appreciated! If nothing shows up I'm going to return it but I like it so much I'd hate to have to!:confused:

No it wasn't :) but the thing is Fry's product page lists no MIR when no MIR is in effect (the one you meant was from 2/1 to 2/25), and right now there is non.

p.s. this form pdf is based on a document scan thats why you were having troubles editing it, the best way to edit it is to add fillable form fields or text boxes on the forms at the parts where you want to add text (I use Adobe Acrobat).
